Specialist dentists diagnose and treat complex conditions affecting the teeth, mouth, jaws and surrounding tissues in a defined area of expertise such as orthodontics, oral surgery, periodontology, endodontics or prosthodontics. You carry out advanced clinical procedures that go beyond the scope of general dental practice, such as placing dental implants, correcting bite irregularities with fixed appliances, performing complex extractions or managing diseases of the supporting structures of the teeth. Much of your working day involves patient consultations, detailed treatment planning, carrying out clinical procedures and reviewing progress over extended treatment courses. You collaborate with general dental practitioners who refer patients to you, as well as with consultants in oral and maxillofacial surgery, orthodontics, and other medical specialties, particularly in HSE hospital settings.
Listen: a day as a Specialist Dentist
You arrive at the specialist dental clinic at 8:15 a.m., ahead of your first patient. You pull up the practice management software to review the day's list: two new consultation appointments, a surgical implant placement, a periodontal review and an orthodontic adjustment session. You check the laboratory reports that came in overnight from the dental technician, confirming that a set of zirconia crowns is ready for a prosthodontic fit later in the week. At 9:00 a.m. your first consultation begins. A GP has referred a 52-year-old man with advanced periodontal disease — chronic inflammation and bone loss around several lower molars. You take a full-mouth periodontal chart, recording pocket depths and bleeding scores, and study the digital radiographs on-screen. You explain the findings clearly, outlining a proposed course of root surface debridement and monitoring, and note everything in the patient record on your clinical software before issuing a report back to the referring dentist.

Entry into dental specialisation in Ireland requires completion of a primary dental degree, the BDS or equivalent, at NFQ Level 8, typically taken at Trinity College Dublin or University College Cork. Following qualification and registration with the Dental Council of Ireland, you must work in general or hospital dentistry before undertaking a recognised postgraduate specialist training programme, usually a Masters or Doctorate at NFQ Level 9 or 10. Specialist training programmes in areas such as orthodontics, oral surgery or periodontology are offered through Irish dental schools and teaching hospitals, often in conjunction with the Royal College of Surgeons in Ireland. On completion of an accredited specialist training programme, you apply to be entered on the Dental Council's specialist register, which is a legal requirement to hold yourself out as a specialist in Ireland. Leaving Certificate subjects that lay the groundwork include Biology, Chemistry and Mathematics at Higher Level, as strong science grades are essential for competitive CAO entry to dentistry.
After graduating with a BDS and completing a period of general dental practice or hospital dental officer posts, you apply for a competitive postgraduate specialist training programme in your chosen field. On gaining entry to the specialist register, you may take up a position as a specialist associate in an established private specialist practice, or as a junior or senior hospital dental officer or consultant in an HSE teaching hospital. With experience, many specialist dentists establish their own specialist referral practices, take on clinical leadership roles within HSE dental departments, or combine clinical work with academic teaching and research at a dental school.
AI-assisted imaging tools are increasingly used in orthodontics and oral surgery to analyse CBCT scans, plan implant placement and design clear aligner treatment, reducing manual planning time and improving accuracy. Digital workflow technologies, including intraoral scanning and computer-aided design and manufacture of prosthetics, are already transforming the prosthodontic and restorative specialties. However, the highly tactile and patient-centred nature of specialist dental procedures means that clinical judgement and manual dexterity remain irreplaceable. Specialist dentists who embrace these technologies and integrate them into their practice will be better placed to deliver efficient, precise care.
